What is NCH MyChart?

NCH MyChart offers patients personalized and secure on-line access to portions of their medical records. It enables you to securely use the Internet to help manage and receive information about your health. With NCH MyChart, you can use the Internet to:

  • Request medical appointments.
  • View your health summary from the NCH MyChart electronic health record.
  • View test results.
  • Request prescription renewals.
  • Access trusted health information resources.
  • Communicate electronically and securely with your NCH Health Care Team.

How do I sign up?

To sign up for NCH MyChart, you must be at least 12 years old. An activation code is required to sign up. There are several different methods of MyChart signup that might be used by different departments across the organization:

  • Clinic staff might sign you up directly while you're in an exam room.
  • You might receive an email or text message with a MyChart activation code.
  • You could also receive a MyChart activation code on your billing statement.
  • You might be able to use self-signup online to create a MyChart account by matching your information against what is on file in your medical record.
  • If you don't have activation code but are already an NCH patient, you can request one online. On the NCH MyChart login page, click the Request One Now button. Fill out the fields and submit. It could take 1-3 business days for processing and verification. Once approved, you will recieve an email with your activation code.

    Once you have your activation code, click on the Sign Up Now button on the NCH MyChart login page. Enter your activation code (not case-sensitive), your date of birth, and your zip code. On the next page, choose the following:

    • MyChart username: Should be something others wouldn't be likely to guess but easy to remember. It cannot be changed at any time and must be between 6 and 20 characters. It is not case-sensitive.
    • Password: Should be a unique combination of numbers and letters, using both uppercase and lowercase letters. Your password must be between 8 and 20 characters and must be different from your MyChart username. At least one number and one letter must be used. Your password is case-sensitive.
    • Security question: This question will be used to verify your identity if you forget your MyChart password.

    When can I see my test results in NCH MyChart?

    Most lab results are released 2 days after they have been resulted. All other results are released 4 days after they have been resulted. Results for patients age 12-17 are limited to non-sensitive lab results. If you have questions regarding your results, please contact your provider directly.

    Why are certain test results not shared electronically via NCH MyChart?

    Not all test results are available in NCH MyChart, including results from outside organizations that are scanned into our system and other scanned results. Your provider is able to determine which types of test results are able to be accessed through NCH MyChart. Further, tests of a very sensitive nature are not released to NCH MyChart. Please contact our Medical Records department if you would like a copy of your results. For more information on how to request records, click here.

    If some of my health information on NCH MyChart is not correct, what should I do?

    Your NCH MyChart information comes directly from your electronic medical record at your doctor's office. Ask your physician to correct any inaccurate information at your next clinic visit. Your health information is reviewed and updated in your electronic medical record each visit.

    If I send a message to my physician, nurse or other provider, when can I expect a reply?

    You will generally receive an answer within 1-2 business days. Please note that NCH MyChart should not be used for urgent situations. Please contact your provider if the situation requires immediate attention or dial 911 if it is an emergency.

    What additional personal health information will the Cures Act now make available to me?

    In addition to the ‘After Visit Summary’ you typically receive when your care visit is complete, you now have access to your physician or care team member’s detailed clinical notes. Users who have proxy access to your NCH MyChart account may also be able to view your clinical notes, depending on their access class. We recommend you review who has proxy access to your account by going to your menu and searching for "Share My Record." In the "Friends and family access" section, you can review your proxy access.

    Where can I access the additional clinical notes?

    You may access the information via NCH MyChart. Login to your account and click ‘Visits’ at the top of the page. Locate your associated visit and click ‘View notes’ to see your provider or care team’s clinical notes. Ambulatory progress notes and Inpatient Discharge summaries are visible starting 4/5/2021. Most other notes are visible starting 6/28/2021. Historical notes are not be visible in NCH MyChart and can be requested from Medical Records using the Request Medical Records option under the menu.

    How do clinical notes differ from my After Visit Summary?

    Clinical notes are medical documents used by physicians and care providers to communicate with each other. These documents include medical language and terminology, abbreviations, and treatment information that may sound technical and at times possibly unfamiliar. In addition, at times, the verbiage may appear blunt or direct. These documents are one tool providers use to communicate relevant information and clinical opinions of the care providers in a way that allows common understanding of the clinical context.

    What if I have questions about my clinical notes?

    Please know that your medical team is available to address any questions you have about your care or health status, as well as that of your child or other proxy’s care. If you have questions that require a short response from your physician, please send a message to your NCH provider via NCH MyChart. If you have many questions and you wish to discuss in greater detail, please schedule a follow up appointment with your provider. This visit would be billed based on the scope of visit and any additional services provided. You can schedule an appointment via NCH MyChart or by contacting your provider’s office.

    What if I would like to have my clinical notes amended or I disagree with statements made on my medical record?

    If you would like to request that an entry in your medical record be amended, you will need to fill out the form found here in order to have your request further reviewed. However, clinical notes are part of your physician(s) and care team's assessment; therefore, any amendment request will be at NCH's discretion based on a review of the note. Factual information will not be amended or removed. You can send the form to Northwest Community Hospital, Attn: Medical records at 800 W. Central Road in Arlington Heights, IL 60005.     How can I access my complete medical history?

    Can I view a family member's health record in NCH MyChart?

    Yes, you can. This is called Proxy access and allows a parent (or guardian) to log into their personal NCH MyChart account, and then connect to information regarding their family member. Complete the appropriate Proxy Request/Authorization Forms located here and return them to your provider's office or the NCH Patient Service Center.

    Can I ask questions regarding a family member from my NCH MyChart account?

    NCH MyChart offers direct access to your personal health record and communicating about another individual's information would be placed in your health record. This information would not appear in the correct health record and could potentially jeopardize medical care.

    How is NCH MyChart secure?

    We take great care to ensure your health information is kept private and secure. Access to information is controlled through secure activation codes, personal usernames, and passwords. Each person controls their password, and the account cannot be accessed without that password. Unlike conventional e-mail, all NCH MyChart messaging is done while you are securely logged on to our website.

    What is your Privacy Policy?

    NCH MyChart is owned and operated by MyChart and is fully compliant with federal and state laws pertaining to your privacy. Your name and e-mail address will be treated with the same care and privacy given your health records and will never be sold or leased by MyChart.

    I was logged out of NCH MyChart, what happened?

    We aim to protect your privacy and security of your information. While logged into NCH MyChart, if your keyboard remains idle for 15 minutes or more, you will be automatically logged out of NCH MyChart. We recommend that you log out of your NCH MyChart session if you need to leave your computer for even a short period of time.

    My activation code does not work, what should I do?

    For your security, your activation code expires after 90 days and is no longer valid after the first time you use it. If you still have problems, call our NCH MyChart Support Line at 847-618-2000.
